Guardian Application

Please fill out the form below as complete as possible. Please be aware of the fields that are required (red asterisks).

Guardians should be 18-70, able to lift 50+ pounds, push a wheelchair for the day and not be a spouse or significant other. Guardians also pay their own way and we do have fundraising opportunities. Current tax deductible fee is $595.
